As we age, the structures of the face and neck lose tone with the loss of collagen that accompanies increasing age. The structures begin to sag following the direction of gravity. In many patients, these early signs of aging begin to show up in the neck. First there will be some laxity just under the chin and this will progress to form two bands that extend from just under the chin all the way down to where the neck meets the chest. In many situations, there is also some fat that accumulates under the chin.
As people age and their laxity increases a surgical procedure that directly tightens the skin and muscles while also contouring the fat, called a neck lift, is recommended. The procedure begins with an incision behind the ear and under the chin, which is easily concealed naturally. During the procedure the fat may be sculpted or redistributed and some of the muscles in the neck will be tightened. The neck lift can be performed by itself or it can be combined with a facelift, eyelid, or brow surgery depending on the situation.
This procedure is usually performed under IV sedation. Patients return home the same day. Most patients return to regular activities in 10 – 14 days.
Typically in younger patients the primary problem is fat; so face liposuction can be used to remove the fat and improve the contour of the face alone. Though patients with increased skin and muscle laxity may need to have an additional procedure to tighten the muscles and skin. One procedure that tightens the skin and melts away some of the excess fat is PrecisionTX™. In addition, Kybella™, a non-surgical injectable, has been approved to melt away the fat under the chin area.
